Neck pain that builds with screens, driving, or sleep position is usually mechanical. A joint in the neck or upper back stops moving well. The muscles above the shoulder blade guard. The head sits forward, and rotation to one side gets shorter. Stretching the trap can feel useful for an hour and then the same spot returns, because the joint underneath never moved.
The visit starts with that pattern, not with a preset adjustment. We ask which side is limited, whether the ache stays in the neck or climbs into the head, and what a workday does to it. Then we check rotation, side bending, and the joints that should share that motion. If a joint is restricted, chiropractic care can restore it. If the tissue will not let the joint go, myofascial release comes first. If a trigger point in the upper trap or scapular muscles keeps resetting, dry needling can quiet it. Those tools can share a physical medicine visit. None of them is automatic.
A neck that only hurts at the end of a screen day often needs the joint and the posture load addressed together. An adjustment without a change in how the head sits tends to fade. Soft-tissue work without joint motion can feel loose in the room and tight on the drive home. That is why this page sits under physical medicine, not as a single technique.
Plan on about 90 minutes for a new physical medicine visit. Follow-ups are about 30–45 minutes. The first part is history: when the neck started limiting, whether headaches travel with it, and what desk, phone, or driving time looks like. We also ask about arm symptoms. Numbness, weakness, or pain that shoots into the arm changes the exam and may need a medical check before routine care.
The exam watches how far you turn, which joint stops the motion, and which muscle is guarding. Treatment that day follows those findings. Some visits are an adjustment and soft tissue. Some are needling of a trap that stretch work has not held. Some are mostly the upper back, because the neck is compensating for a stiff thoracic spine. You should leave knowing which of those it was, and what the next visit is for if the same limit returns.
